What is the UMID Card?
The Unified Multi-Purpose ID (UMID) is a government-issued ID in the Philippines that combines several government IDs into one convenient card. Issued by SSS, GSIS, and PhilHealth, the UMID card contains biometric information such as fingerprints, digital signatures, and photographs. It serves as a valid ID for government transactions, healthcare services, and banking purposes.
Who Can Apply for a UMID Card?
Eligible applicants include:
- SSS members with at least one contribution posted
- GSIS members or pensioners
- PhilHealth members
- Filipino citizens needing a government-issued identification card
UMID Card Application Requirements
For SSS Members:
- UMID Card Application Form – Available online or at SSS branches
- One primary ID or two secondary IDs
- Primary IDs: Passport, Driver’s License, Voter’s ID, Postal ID, Seaman’s Book
- Secondary IDs: Company ID, School ID, Birth Certificate, Barangay ID
- Copy of Birth Certificate – Issued by PSA
- Copy of Marriage Certificate – If applicable
For GSIS Members:
- UMID Card Application Form – Available at GSIS offices or online
- Two valid government-issued IDs
- Updated GSIS Membership Record
Step-by-Step UMID Card Application
Step 1: Prepare Your Documents
Ensure all IDs, certificates, and forms are complete and accurate to avoid delays.
Step 2: Visit an Enrollment Center
- SSS Members: Schedule online through SSS or walk in to an enrollment branch
- GSIS Members: Visit the nearest GSIS office for submission
Step 3: Biometric Enrollment
Submit your documents and undergo biometric data capture including photograph, fingerprints, and signature.
Step 4: Processing and Card Release
Processing usually takes 4–6 weeks. You will receive a notification once your UMID card is ready for pickup. Claim the card at the branch where you applied.
